Coordinates the completion of the MDS process with the interdisciplinary team and monitoring of the facility QI indicators.  Coordinates and schedules careplan conferences with staff, residents and responsible parties in coordination with social services.  Develops and implements resident careplans and ensures that the careplan interventions are followed by the appropriate disciplines.  Serves as the chairperson for the falls program/committee.  Monitors and intervenes with weight loss and the completion of monthly weights on all residents in the facility.  Assures the compliance of the facility with the infection control procedures involving Heptovax, PPD, Flu Vaccine and immunizations.  Coordinates with the Director of Nursing to ensure the quality of nursing care is provided to each and every resident in accordance with current federal, state and local regulations and facility policies.
